Accessing medical records - Your rights

Request for access to records  - This request form is being phased out. Please ask reception to place you on the list for an administrator to give you a call to set up 2nd Level access to your clinical notes . This will enable you to book GP appointments, order repeat prescriptions and view your clinical notes for yourself.
